At BoldAge PACE, your satisfaction and well-being are our top priorities. If you are ever unhappy with the care or services you receive—or if you disagree with a decision we’ve made regarding your care—you have the right to file a grievance or appeal at any time.
You may express your concerns verbally or in writing. The process is outlined in your Enrollment Agreement, and our team is always here to help. Please speak with any staff member or call your local BoldAge PACE center—we will guide you every step of the way.
Appointing a Representative
If you would like someone to file a grievance or appeal on your behalf, you may appoint a representative. This could be a family member, friend, caregiver, or someone you trust.
To appoint a representative, you must complete and sign the CMS Appointment of Representative Form (Form CMS-1696). You can download the form here: CMS Form 1696 – Appointment of Representative (PDF)
Once completed, please return the form to your local BoldAge PACE center or give it to your care team.
