BoldAge PACE Engagement Navigators will guide you through the enrollment process. They will listen to your needs, answer questions about our services, give a tour of the center, introduce you to providers, and help you understand if PACE is right for you.

Our team will meet with you to understand your needs and determine if you qualify for PACE. Together, we’ll explore how BoldAge PACE can support you in living safely and independently at home, in line with state requirements.

Our BoldAge PACE Engagement Navigators will submit your information to the state for approval into the PACE program. Upon approval, we will finalize your enrollment so you can enroll on the first day of the following month. Should your application be denied, we are here to help with the state’s appeals process.

Your BoldAge PACE Engagement Navigator will guide you through the enrollment agreement and any necessary paperwork. We’ll review what to expect, work with you to create your interim care plan, and schedule your first visit to the PACE center — ensuring you are fully prepared for this next step in your care journey.
