What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) provides healthcare and support services to help older adults live independently and safely at home.

Participants must receive all healthcare services, including primary and specialty care (other than emergency services), from BoldAge PACE or an authorized entity. Participants may be personally and fully responsible for unauthorized or non-PACE services.

What is PACE?

PACE, the Program of All-Inclusive Care for the Elderly, is a Medicare and Medicaid program that helps older adults get the care they need in their homes and community. As a PACE participant, you must receive all healthcare services, including primary and specialty care (other than emergency services), from BoldAge PACE or an authorized entity. You may be personally and fully responsible for unauthorized or non-PACE services.

Who is eligible for BoldAge PACE?

PACE is designed for individuals who are 55+; are able to live safely at home with support of PACE services at the time of enrollment; live within the BoldAge PACE service area; and qualify for nursing facility level of care as determined by your state.

Why enroll in a BoldAge PACE program?

PACE provides you and your family with multiple layers of support that enable you to live at home, where you want to be. With services designed around your needs and personal goals, you can live your life the way you want to, with us by your side.

How much does it cost?

Once enrolled in PACE, there are no deductibles, copays, or benefit limits for any prescription medications, medical care, or services approved by your PACE care team. How much you pay is determined by your financial situation. If you qualify for Medicaid, or are dually eligible for Medicaid and Medicare, you may pay no premium for the PACE program. If your income exceeds a state-determined amount, you may be responsible for a monthly share of cost or patient liability.

How do I enroll with BoldAge PACE?

If you are 55+ and live in a BoldAge PACE service area, reach out to us at 855.801.BOLD (2653) or fill out this form. The BoldAge PACE team will visit you to determine your needs and eligibility, and help you enroll into PACE. If you have questions about whether you qualify for Medicaid or need assistance applying for Medicaid, our benefits’ specialist can assist.

What can I expect after I enroll?

You will meet with your dedicated care team to develop a personalized care plan that promotes your happiness and well-being. Our drivers will take you to doctor’s appointments, our PACE center will be available for activities and meals, and you’ll have a dedicated partner in your health journey.

What services are provided by BoldAge PACE?

PACE services include all your medical care, vision, dental, social services, rides to doctors and the PACE center, nutritional services, help at home, fun activities, and so much more. Learn more about the services BoldAge PACE provides here.

What if I need to see a specialist in the community?

If your BoldAge PACE team cannot directly provide a specific medical service, we will authorize services to see a specialist. We will schedule the appointment, provide rides to your appointment, and follow up with the specialist to ensure you get the care you need.

Does anyone live at the BoldAge PACE center?

PACE is designed to support you so that you can live independently in your home. Our PACE centers are open Monday through Friday during normal business hours, so participants head home at the end of the day. For people who need additional care, we can provide help at home.

What if a PACE program is not for me?

If PACE isn’t for you, you may voluntarily disenroll. Those with Medicare or Medicaid will be assisted in returning to their former healthcare coverage. A participant will never be disenrolled due to changes in health status.

Does BoldAge PACE become my new insurance provider?

When you enroll in BoldAge PACE, you agree to receive all your healthcare services through our program. We have our own team of providers and contract with a network of healthcare professionals to ensure all your medical needs are met.

BoldAge PACE replaces your insurance, coordinating all your healthcare services, managing appointments, and handling claims on your behalf. While BoldAge PACE guarantees access to all necessary services, we do not guarantee access to a specific provider. Our care team will ensure you receive the right services through our PACE center and provider network, delivering high-quality, personalized care.

How do I file a grievance or appeal?

At BoldAge PACE, your satisfaction and well-being are our top priorities. If you are ever unhappy with the care or services you receive—or if you disagree with a decision we’ve made regarding your care—you have the right to file a grievance or appeal at any time.

You may express your concerns verbally or in writing. The process is outlined in your Enrollment Agreement, and our team is always here to help. Please speak with any staff member or call your local BoldAge PACE center—we will guide you every step of the way.

Appointing a Representative

If you would like someone to file a grievance or appeal on your behalf, you may appoint a representative. This could be a family member, friend, caregiver, or someone you trust.

Once completed, please return the form to your local BoldAge PACE center or give it to your care team.
