BoldAge PACE is proud to announce the opening of its newest center in Crestwood, Illinois, located at 5419 W 135th Street, Crestwood, IL 60418. This exciting expansion brings the nationally recognized healthcare to Cook County, giving more older adults access to the care and connection they need to live safely and independently at home.

With strong support from local and state leadership, including the Mayor of Crestwood and Representative Thaddeus Jones, this expansion reflects a shared commitment to improving access to quality care for older adults in the region. Their advocacy and partnership have been vital in making this milestone possible for Cook County families.
What BoldAge PACE Offers
BoldAge PACE provides complete medical and social support designed to help older adults remain at home, living life to the fullest. Participants receive care from a team of doctors, nurses and therapists who work together to manage every aspect of their health. The program also offers:
- Full head-to-toe medical care
- Free rides to medical appointments and to the day center
- A welcoming activities center where participants can meet friends, enjoy warm meals, and take part in engaging programs
- In-home support to help participants stay safe and comfortable in their own homes
Most importantly it’s coordinated by one care team with a whole lot of love! This ensures that every participant has access to complete care and the community connection that makes a difference in their daily lives.
Leadership and Experience
Leading the new Crestwood center is Executive Director Wanda Cooper, who brings 20 years of experience in PACE leadership. Wanda’s expertise and deep compassion for older adults have been instrumental in launching and guiding programs that support aging in place with excellence and care.“I’m honored to support the Crestwood team,” said Cooper. “Our mission is to help older adults live full and connected lives at home, with everything they need under one roof.
